In 2004 I wrote in Why Our Schools Are Failing, based on the 1996 literacy test, 27% of year 3 and 30% of year 5 students failed.  The results just as bad in numeracy.  Nothing new in today’s headlines but those responsible have never been held to account.  Go figure.
