The NASEM approach of having systematic reviews conducted by a clearinghouse that go on for an expert committee's review in an iterative back & forth process should be the gold standard. Folks really need to stop worshipping at the feet of a Cochrane systematic review

RCTs of complex interventions really need way more scrutiny in EBM. Often systematic reviews and meta analyses have absurdly broad inclusion criteria that might involve similar key words but ultimately capture very different interventions. Happens in nutrition all the time
