1/n SIADH is the FSGS of electrolyte disorders. Why? because it represents a pattern of disorder of water regulation (⬆️uOsm, ⬆️uNa) that can be triggered by different mechanisms of disease, like FSGS is a pattern of injury that can be the result of various glomerular hits.

1/n Time for a #UroNephrology case. Let’s start by discussing MAG3 nuclear scan, a fairly underutilized test in nephrology. What is MAG3? It’s a 263 D molecule that is both filtered and secreted through the kidney, thus, it’s a marker of total renal perfusion / renal blood flow.

2/n it can be useful to asses split renal perfusion and to rule out obstruction, for the latter, the ideal test is a NM diuretic renogram. Unlike MAG3, DTPA is a tracer for assessment of GFR. Let’s go over the GIF again:
