Anyone that doesn't know what a session in GP is or thinks a 3day week in GP is part-time, read this: Recently I went from 9 clinical sessions to 7. I still work 9/week but 2 are in a different role now. It's still pretty full on but 9 clinical sessions was going to break me.

Yep. Smaller practices like mine have little to no contact with the commissioners. Rural practices often have significant deprivation & don’t benefit from local initiatives as they are rural. Meanwhile ICB leads continue to throw down that we are “the only practice to ever…”

Sharing patient feedback from somewhere in the UK’s NHS, who had a lengthy hospital stay: “The staff worked so hard, but it was like watching people putting out fires with buckets of water. Nothing seemed to work and the public has a glossy facade of an NHS appearing to cope”

I don’t like emphasis on a “predominantly female workforce many working pet time” (paraphrase) ⬇️ - it ignores fact that GP these days is so intense that hardly anyone can manage FT. But also it’s c30yrs since women > men graduating from med school. There’s been time to address!

What a kick in the teeth to these local GPs who tried their best to keep the practice going, had to throw in the towel only to see their local commissioners hand the contract over with a huge uplift of funding. Wonder if Wes Streeting MP is aware… pulsetoday.co.uk/news/practice-…

The ICB couldn't support the existing local GPs, however they have managed to find another £40/patient for a private provider to do the work instead. That is a massive uplift! Massive. Wes Streeting MP please do take a look at what is happening up and down the country.

pulsetoday.co.uk/news/practice-… With an extra £40 per patient (and a 1% annual NHS budget reallocation), General Practice could be transformed. Prevention ✔️ Quality outcomes ✔️ GP-led care ✔️ Patient satisfaction ✔️ Healthcare to be proud of ✔️ Partners know how to do this – NHS

I wonder what kind of setup they have to collect 15GB of data and then analyse it. TensorFlow? PyTorch with GPU acceleration? How does it self-learn and improve it's "diagnostic ability"? 50 million data points...impressive. I want to know more.

GMC failings GMC had become unsafe for Drs from ethnic minorities with a track record of bias It has ‘leaders’ who accepted pressure to take on regulation of PAs/AAs despite being the ‘Independent Regulator of Doctors’ Now hiding data from a half hearted survey on PA/AA roles

Pt seen in Resp clinic for bronchiectasis last week. Clinic letter ✉️ ⬇️ . Told to see GP. 🤦🏽‍♂️. So sh!t & lazy. It’s literally your job to make recommendations on which ABx according to sputum culture you 🫵🏽 should have arranged in clinic. Poor pt is stuck in the middle.

Mooting the replacement of GPs in primary care with focus on rural and remote areas. Relevant given the recent FT analysis demonstrating the 2-tier NHS system emerging whereby most deprived areas get PAs whilst the least get GPs 4/ ft.com/content/5a5335… pulsetoday.co.uk/analysis/speci…

There are some fantastic resources on safe working here: bma.org.uk/advice-and-sup… If you are a GP or PM the full guide should be a must read (and great for appraisals too)!
