Drawing: Possible causes of your problems. I've been appalled at the scenes we've witnessed over the last few days. It's utterly unacceptable that people should have to live in fear because of the colour of their skin. [Polite comments only, please.]

I've posted about this before, but: No beds in 🏥 or access to alternatives 👇👇👇 Long delays in 🚑 handover at A&E 👇👇👇 High 🚑 cat 2 response times 🤔 The solution for high acuity response times is not at the front end of the system hsj.co.uk/acute-care/rev…

We have poorly, worried and anxious people trying to help poorly, worried and anxious people. It's not working. Every bright young "consultant"about to speak at me is just getting a copy of this.

We lobbied so hard for NHSE to routinely publish 12 hour length of stay times (from time of arrival) and this is why. We are pleased that the PM acknowledges the avoidable deaths caused by long stays. Let's fix this together with #resuscitateemergencycare

Fun, but also vitally important for a sustainable job plan in EM. But the powers that be see minors as easy, cheap work to be farmed off to the lowest quality and cheapest provider… Big mistake.

Misbah (مصباح) Catherine Williams Royal College of Emergency Medicine EMTA It is a very big concern - who are the ENPs going to ask for help? What happens at night when there isn’t a 24/7 Minors service? (20+ years ago occasional EDs used to send minor injuries away to come back in the morning but those days are surely gone?)

8/ WHERE POSSIBLE do not allow the failure of emergency departments to lead to failure of core emergency medicine - although this one is the hardest

ED crowding: anyone know of any papers/data on the reduction in productivity (and hence increased patient processing time) that comes from EDs being physically full? We all know a vicious cycle starts when you can’t find anywhere to see patients, but has it been quantified?

The SHED study is published! Thanks to the 100s of contributors around the UK... This paper raises a lot of questions - how will it influence your work? What post test probability of SAH and aneurysmal SAH are you, and your patients, comfortable with? emj.bmj.com/content/early/…

Dr Linda Dykes @[email protected] PAULTACG We did do some pilot work recently as a student project, got quite granular (and I hate myself for saying that) 9% of clinician time recorded was "waiting for EPR to load". Andrew Tabner Poster at ASC (if you're going). And clearly we have nothing to compare it with.

rcem.ac.uk/roadmap-to-rec… We engage with all parties and offer guidance as to how to make emergency care better. This is a fixable problem, but requires committment, transparency and honesty.

Cardiac arrests are time critical clinical events Female Drs shouldn’t have to worry abut appearing ‘fierce’ when they are trying to lead They shouldn’t be routinely assumed to be less experienced than male colleagues in the room This is excellent work, but depressing to read

Surgeons wouldn’t operate in a broom cupboard Dermatologists wouldn’t run clinic in the canteen ICU wouldn’t ventilate patients in the car park So why do we expect that Emergency care can be provided on corridors?
