Streptococcus anginosus (formerly milleri) group

• 3 species: S.anginosus, S.constellatus, S.intermedius
• Colonize oral cavity/GI/GU tract
• Prone to cause abscesses incl brain, esp S.intermedius
• Tiny colonies with butterscotch/caramel odor
• Usually penicillin-S

Melioidosis - the great mimicker

• Caused by Burkholderia pseudomallei, a GNB found in water and soil
• Endemic in SE Asia and N Australia
• Dx by culture (serology not reliable)
• Treatment: intensive (ceftazidime or meropenem) + eradication phase (TMPSMX or Augmentin)
