#realisticmedicine, #futurecareplanning must have person centred outcomes at its core....and do we need to talk about the possibility of requiring #care as a key component of this. For many, talking about moving to a #carehome with loved ones, is a tumbleweed moment.

Lovely to have the husband of a former resident pop in today. He likes to keep in touch. We spoke about his wife, how his health is, football....he is still very much in his bereavement journey, his connection to the care home is important - friendly faces, familiarity, an anchor
