I think mental health notes are pointless and the massive amounts of unnecessary information collected makes patient care unsafe. If staff actually read them they get lost in years of other people's opinions, which are presented as fact, and facts which are often 1/

How cool would it be to do a small study with a group of mh pts, where their notes were condensed into one document, (the rest made inaccessible to staff) & they received care as normal. Staff & pts could report their experiences before, during & after, to see if care changed.
