Thomas Dekkers | Spinal Specialist Physiotherapist I was fortunate to see removal of that during surgery a month ago. Fatty lump causing deformation of a nerve root. Looked very similar to the one presented here.

Space occupying lesion affecting the PLL & dura mater. Doubt if neoplastic but defer to radiology. That's a T2 image & signal intensity is strong, but not = CSF. Either disc material or fat. Adjacent L4/5 disc narrowed so probable past disc herniation so maybe origin?

Mark Laslett Yes intriguing questions Mark. I’ve never seen such a fat deposit inside the posterior longitudinal ligament before. Not sure if disc tissue can morph into fat? Maybe fat infiltration like muscle?? intriguing…. 🧐
