Dear ACL docs We don't think the QT is the future of ACL reconstruction. It's a viable option but it is NOT the future. If it is, then we need to reconsider our RTP protocols and better understand how to rehab it because we PT's (many of us!) dislike it as a graft option.

Lenny Macrina, Sports Physical Therapist We’ve learned a lot: ✳️surgeons should not take huge grafts & ✳️don’t get into muscle ✳️need early quad activation (often hard) ✳️aggressive early ROM (lack of full extension early➡️long, tough recovery) ✅great graft option but nothing is perfect, still needs a lot done right

Lenny Macrina, Sports Physical Therapist We did a ~45 month follow up in a small cohort of QTs and HTs. SLCMJ height and SLDJ reactive strength still impaired in the QTs, compared to the hams. Not sure on the re-tears in either group but I’m sure we’ll look at it!

It's amazing to me how infrequently I see a loss of shoulder IR in baseball pitchers and how often I see excessive ER. Yet GIRD is still something I hear talked about all the time. This is a big reason for all the Tommy John injuries.

I golfed with a 1 hcp yesterday and watched some very boring golf. 14/15 in fairways hit 18/18 GIR burned the hole numerous times but mainly had tap in putts Every driver swing was like watching a robot swing... no thinking over the ball. No birdies but numerous close calls He
