Wearing a helmet while riding ATVs is a MUST. Riding in uneven terrain at a high speed can result in serious injury. Open depressed skull fx with dural lac and brain egress. Required I&D/crani, frontal sinus exent, and EVD. We have an amazing plastics, ophthalmology, trauma team!

7M with MIE secondary to a right perinatal MCA infarct. Hemispherectomy in these kids are great surgeries -many times they have already functionally remapped with minimal deficits post op. Technically easier/faster, faster recovery, higher success scores. Let’s cure epilepsy!

Resecting the amygdala is the hardest to visualize when you are new to ATL, as it doesn’t have complete pial anatomical boundaries. Look for the MCA bifurcation superomedially, the third nerve inferiorly, and the inferior choroidal point posteriorly.

While sEEG has rapidly become our “go to” option for localization, grids still have a role - particularly in mapping. This is a pt with a left frontal dysplasia radiographically abutting Broca. While you can map with sEEG, imho grids give you greater spatial coverage.

3M with right hemimegalencephaly and MIE, seizing multiple times daily. IMHO these are the most technically nuanced hemispherectomies, imperative to manage blood loss and complex anatomy that is many times atypical. Making sure you did all the disconnections requires patience.
