Agree to all of this, esp final point about resource investment to improve RCTs

Randomized trials don’t have to be as costly or as complicated to pull off as they are. We made them that way, and we can make them better & easier to do.

But only if we’re willing to try.

Looking at all these reasonable and not-unexpected deviations from the patients' allocated treatment regime and wondering when I get to be involved in one of these clinical trials that *doesn't* take place in the 'real-world'....

True story: when we were setting up REGAIN, a patient partner told us that her ability to bounce back from adversity made the difference in her recovery from her hip fracture at age 95.
