At this point I’m convinced that most medical societies start with good intentions but eventually morph into self fulfilling organizations whose goals are to exist, expand, and make money.

Part 2. I have it in all colors. The H&E shows what seems to be a multinucleated giant cell. Yes, it is AMYLOID. The frozen section allowed a precise diagnosis. The macroscopic appearance was also very impressive (brown, granular, crumbly)

Sanjay Mukhopadhyay Pepe Jiménez Heffernan Samson W. Fine, MD Sinchita Roy MD PhD Angel Panizo MD, PhD 🇪🇸 🔬 Ankur Sangoi Lola Lozano Andrew Bellizzi 🇺🇦 Co-expression of SOX11 (often strong and diffuse) and CK20 in neuroendocrine CA almost always favor diagnosis of MCC. Focal expression of SOX11 and the lack of CK20 expression may suggest non-MCC (e.g., pulmonary small cell CA).

Sanjay Mukhopadhyay Pepe Jiménez Heffernan Samson W. Fine, MD Sinchita Roy MD PhD Angel Panizo MD, PhD 🇪🇸 🔬 Ankur Sangoi Lola Lozano Andrew Bellizzi 🇺🇦 Co-expression of TRPS1 and INSM1 in skin tumor invariably suggests diagnosis of EMPSGC. In contrast, MCC is consistently negative for TRPS1 while showing immunoreactivity for the latter.

The Phlegmfighter Scott Kilpatrick, MD Sanjay Mukhopadhyay A pathologist is almost always an author (an usually the last author) in the case records of the Massachusetts General Hospital. This has been a tradition since the CPCs were initiated by Richard Cabot >100 years ago. The Phlegmfighter you can check that in the NEJM NEJMCPC wsites

Sanjay Mukhopadhyay Pepe Jiménez Heffernan Samson W. Fine, MD Sinchita Roy MD PhD Angel Panizo MD, PhD 🇪🇸 🔬 Ankur Sangoi Lola Lozano Andrew Bellizzi 🇺🇦 Diffuse positivity for neuroendocrine markers and keratins almost always indicate an epithelial neuroendocrine neoplasia. Positivity for the former but absent stain for the latter strongly suggests pheo/paraganglioma.

CC325. Alone in the office. Mediastinal EBUS-TBNA with suspected sarcoidosis. Granulomas⬇️. Everything is ok. Time has taught me that in these cases it is useful to see if there is an oncologic history, whatever it is, retrieve the slides and check them. And that is what I did.
