This is fair enough, but there is an opportunity cost. In the 10 years post ACA, CMS was focused implementing ACOs and other parts of the value strategy (e.g. MIPS). Researchers focused on evaluation. MA grew with relatively little scrutiny from policy community at this time.

Agree that squeezing physician payment forever is not the path to long term Medicare sustainability. Most important issue is use of new tech. MA adoption of biosimilars is an example of how incentives can lead to lower cost substitution.

We estimated savings in the MSSP using your treatment effect from the NEJM paper. Gross savings exceeded incentive payments through 2018. This changed 2019, leading to net losses over the life of the program.
