Interesting Colorectal Disease study from our colleagues across the pond that explores an exceptionally important topic: the impact that have on the surgeon's wellness and future decision-making.

Which is what we see in Pitstop, surveys, etc.

1) It showed fairly good agreement between groups. Agreement between photograph raters was higher than between those assessing from a photograph and those assessing in real life.

And this shows you how it compares to other classification systems. Almost all look at laterality, recurrence, and some look at associated abscess. All look at similar ideas, just measured differently. It suggests the tool has some face validity.

We wanted to know 2 more things about this tool:
1) Did raters classify cases consistently?
-By asking surgeons to classify photos from the cohort.
2) Did classification correlate with ?#
-By exploring class & baseline and post-treatment PROMs

This is the International Pilonidal Society (IPS) system we used in this study, primary vs recurrent and pit extension were key domains.

How do you classify pilonidal disease (or any surgical disease)? Chances are you are using a tool with limited validity. In PITSTOP Study we wanted to look at how well a tool performed...

In summary, we think the IPS tool shows:
✅Face validity
✅Inter-rater reliability
✅Concurrent validity

If we are going to address the challenges in the literature, adoption of this classification (now tested), should be considered.
