The L5-S1 paraspinal approach has rescued me quite a few times especially in the elderly with age-related degenerative changes in back anatomy & gait. I was reminded of it by an excellent teacher Dr Robbie Erskine more a decade ago when I was working wih him as his resident 🙂🙏🏽.

They vary together. You can “bite on a stick” to reduce opioids but that’s probably not the best for recovery.
Better yet we need to move past 24h outcome and look at function ->

I could see an argument for ‘double dose Dex’ but I’ve never seen anyone give 4 vials a la Dr Robbie Erskine.
