The only exception to the rule below: you can (and should) ‘nail’ the *aortocoronary* ostium, if you use a ‘floating wire’ in the aorta a get your IVUS work done right. See this case of absent left main with independent LCx & LAD ostia. Atherectomy & stent flush at the LCx ostium

1/3 of pts undergoing elective/urgent PCI have ⬆️ #LVEDP ➡️ higher-risk profile. Death was 2x ⬆️ in pts with LVEDP >18 mmHg. LVEDP was an independent predictor of death, AKI, new HF and longer stay.

Thanks Manuel Pan & Soledad Ojeda for the invitation to perform a live case at the #OMG2022 course in lovely Cordoba 🇪🇸. This was a challenging RCA #CTO (J-CTO 3, both caps ambiguous) requiring the retrograde approach (reverse CART + tip-in) after antegrade extraplaque tracking.
