Teaching subepithelial deposits (humps).

1) Infection-related GN (top panel)
2) C3 glomerulopathy
3) Proliferative GN due monoclonal Ig (bottom panel)
4) Lupus (Autoimmune) GN

Humps likely reflect complement.

Subepithelial location prevents removal & humps persist a long time.

The diagnosis and systematic work up of antigens in MN.

What we now need are serologic tests to detect serum antibodies to the antigens. I do hope that they will become available in the next 3-5 years

Sometimes a simple stain can make a pathologist’s day. In this case, positive NELL1 in a case of membranous nephropathy today.

Dx: NELL1-associated MN.

79 yr-old smoker with 2 grams proteinuria. Cr 1.7

Will need malignancy work up, as NELL1-MN often associated with malignancy.

Great teaching case: Cryoglobunemic GN, type 1.

LM: membranoproliferative GN pattern, with intraluminal and capillary wall deposits (trichrome and PAS + silver-)

IF: IgG lambda, kappa -

EM: intraluminal and subendothelial deposits with tubular substructure
