Thank you Fran Ives for the opportunity to talk about life before #PSIRF, of course with some PSIRF mixed in. And now for the realisation that other people might listen... Would definitely recommend checking out this series - lots of great guests and #humanfactors content

Great question and responses. Although it implies that #PSIRF is a static thing. It’s more likely that how the principles are applied will change over time. It offers an opportunity to continue to evolve, learn and improve

All Systems Ergo - episode 2 Great to chat to Eva-Maria Carman & Giulia Miles from Trent Simulation Nottingham University Hospitals on their HF work. Not one, but two HF Specialists! spotifyanchor-web.app.link/e/5okP4pXpNHb #allsystemsergo #humanfactors #ergonomics

Just over a week until the proposal is in for PSIRF safety prompt card idea. We would appreciate any ideas on how this could be improved, any iterations needed, or just how you think this could help you! Please comment on the Q website here: q.health.org.uk/idea/2024/psir…

📢WE ARE RECRUITING for a Senior Research Fellow to join our de-cluttering for safety theme. Do you have experience in research project management and conducting high quality, health services or applied health qualitative, quantitative or mixed methods research?

Enjoying #Quality2024 so far, particularly this book club with Amy Edmondson. Mentioned it to my daughter (who also attended the conference on Weds) - she has asked for a book club with J.K. Rowling

Is linear, complex and intelligent a better categorisation? Failing to de-ice a plane is not human error nor basic. It is a symptom of a complex system problem. Struggling to see the benefit of categorising failure as basic #Quality2024

Also wondering about the impact of using the term ‘failure’ and this work from Steven Shorrock on setting language/taxonomies to neutral humanisticsystems.com/2023/11/11/set…

A great opportunity to review and comment on HSSIB’s investigation criteria. Have seen similar concepts in local patient safety incident response plans #PSIRF

It was not a short journey, but I have learned a lot by working on this book element. Thanks to the co-authors and THIS Institute for pushing us think about how to make this piece more evidence-based.
