Iain Moppett We have a protocol with written instructions for the different mixes stuck literally on the OR wall, that must be checked off by the theater nurse. For day surgery (WALANT mainly) the mix is done by the hospital pharmacists and sent to the day surgery area, just like for chemo.
Iain Moppett Most common “pharmaceutical” error I’ve seen - without consequence fortunately - using distilled water instead of saline for arthroscopy and cystoscopy. Noticed when cautery doesn’t work because distilled water is non-conductive. 3L bags are THE SAME. We talked to the company.
Iain Moppett Company said they wouldn’t change the packaging so the safety committee has proposed to use a different brand for each type of 3L bag.
