Closure of POEM mucosal entry correctly is v. important. Here are my tips for a proper closure.
First clip in the most important to get a clean line of clips ensuring no chance for leak

I use double scope method at POEM routinely on advisement by my former co-fellow at SMH Gastroenterology and now mentor A/Prof Yuto Shimamura
Pro: helps delineate line of myotomy as well as the length of gastric myotomy+helps reduce rate of inadequate myotomy
Only con: needs 2nd operator

Many uses of endoloop cutter! Young pt comes in with bike accident and traumatic D2 perf. Surgeons repaired with NJ insertion.
NJ unable to be removed (pt c/o 'pulling' sensation+pain).
