Followup:

A percutaneous drain was placed by IR as a marker. Via a 4 cm flank incision between ribs and using a laparoscope to aid visualization, the abscess was drained and stones extracted.

Imaging showed complete resolution, and there has been no recurrence in 7+ years.😎

A patient presents 3 months after laparoscopic cholecystectomy in which there were numerous spilled stones.

CT shows a phlegmon in a very superior and posterior subphrenic location (see images). The patient is moderately septic.
