🔥SVT Quest🔥

For EP beginners, remembering the diagnostic criteria for SVTs like AT, ORT via an AP, and AVNRT can be quite tricky😱
There’s a ton of jargon—terms like V-A-V, V-A-A-V, V-V-A responses, orthodromic His capture, total pacing prematurity, A2-H*-V*, and more—plus…

Strange pathway( left anteroseptal) with intermittent antegrade conduction and repeated AVRT after 3 times failed RFA; was ablated successfully in LCC! with this signal 👇gone in distal CS/ AS groove but returned immediately !

When a long-term memory forms, some brain cells experience a rush of electrical activity so strong that it snaps their DNA.

Then, an inflammatory response kicks in, repairing this damage and helping to cement the memory, a study in mice shows go.nature.com/43DZQns

TogetherAgain posters

High-power bipolar ablation of using two 8mm catheters. Tissue thicknes 15-40mm 🔥

External irrigation provided by sheaths advanced the level of chateter electrodes🚿

Dense lesions up to 20mm, borderline lesions 23mm📏

Mahaim postulated the presence of a “praspecific” system of
atrioventricular conduction

Whereby the AV axis may not be fully insulated from the underlying ventricular myocardium

He called them “superior septal
connections” – ie fasciculo-ventricular connections

Curious development of the atrioventricular conduction:

Tawara’s famous histology slide of the atrioventricular conduction axis from 1906

Purple = insulating tissues of the atrioventricular junctions

Yellow = myocardium

Orange = conduction axis

Atypical atrial flutters in pts w/out prior surgery or complex ablation are often due to localized re-entry. Other ATs commonly occur during long-term follow-up following ablation, suggesting progressive atrial myopathy in these pts. bit.ly/3VaDasL

9 yom pt w/manifest RFW WPW
Succesful RFA. AP recurrence after 30 mins waiting time & sheaths removal.
New groin punctures.
Now, only Dormant preexcitation seen w/adenosine bolus.
Succesful RFA after 3rd attempt with this approach.
Never done
