A shame—though the IRB may not be to blame. Their job is to ensure the study is safe—and the younger or healthier the subjects, the safer it may seem.

But reviewers, journals, & payors should object when a trial is so unrepresentative it yields no useful safety data for others.

What is the greater driver of antibiotic overtreatment for recurrent UTI in real-world settings—the lack of antibiotic-sparing prevention strategies, or the confusion about when and how clinicians should diagnose and initiate treatment for suspected UTI? tinyurl.com/3y9xznf8

🔥🔥🔥Important work in JAMA Network Open! We found inpatients with ASB rarely had bacteremia--pts w/altered mental status and NO systemic signs of infection had <<1% risk. Really drives home that work up of other causes of altered mentation is key, rather than anchoring on UTI

Our JAGS study on overlapping caregiving and care-receiving in older US adults: rather than making binary distinctions between caregivers and care-receivers, clinicians and policymakers should acknowledge the fundamental overlap and dynamic nature of these roles as people age.

Great article. Some of my early mentors are retired, but I still find myself reaching out when I have something on my mind.

My trust in them has endured even if our relationship has changed. It’s a gift to have mentors who are wise, generous, and thought-provoking all at once.

Alas, it’s hard for LoRs to get more useless, with so many academics now drafting the LoRs they need themselves (if junior) or paying staff to write them (if senior). Maybe the day will come when one AI program writes the LoRs, another interprets them, and we avoid them entirely!
