All people with asthma and COPD should have a self-management plan when they leave hospital to help them seek support sooner rather than later, and actively participate in their wellbeing. Everyone should have the tools and resources to live well with a respiratory condition.

Pulmonary rehabilitation is highlight effective for people with COPD and other lung conditions. All patients requiring PR should have timely access, and this means creating increased capacity to ensure patients can start their course quickly in line with national standards.
