Hey insurance companies and pharmacy benefit managers and pharmacies -- requiring a prior approval and inflating the charge for a cheap generic drug makes zero sense!

It's bad for patients and is unethical.

These reports of ceftriaxone-resistant gonorrhea are appropriately receiving much attention.
