Persistently painful L1 VCF w/ edema on STIR after an initial Kyphoplasty revised with SpineJack. An example of how the reamer can penetrate a previously cemented vertebral body. The sagittal vertebral angle decreased 5 degrees afterwards. ASPN ASIPP Society of Interventional Radiology

Sclerotic fractures on CT are often described as 'remote' or 'chronic' because it's something that can be said pretty confidently. Unfortunately this descriptor sometimes ignores other salient features, such as a hypotrophic nonunion cleft with pain >6 months after injury.

Even with SpineJack, cement must fill across the midline of the vertebral body. If it isn't behaving (rarely happens), a high parapedicular access is a great way to add that necessary cement to the center.
bipedicular Jacks + uniparapedicular access = baby!
