A very thoughtful and thought-provoking piece and an excellent tweetorial as well. Shocking stats- while >80% of RT trials have OS primary endpoint, only 32% of systemic therapy trials have OS as the primary endpoint. Just think about it.

This is surprising – was expecting FFX arm to win!

Compared to PREOPANC-1, both arms did better, including the RT arm (that got exact same regimen as in P1).

Look forward to presentation – breakdown of borderline/resectable, toxicity/QOL outcomes, patterns of recurrence.

For AR or BR-PDAC, neoadjuvant FFX and neoadjuvant Gemcitabine based CRT (36 Gy/15 fx) had comparable outcomes.

mOS was 21.9 m with FFX arm vs 21.3 m with CRT (HR 0.87; 95% CI 0.68-1.12, p=0.28), and similar resection rates 77% vs. 75%.
