Analysis of 80 potentially futile trauma transfers – goals of care are discussed in only 10% of them! How can we be better about talking to patients / families about prognosis and goals early on?

Was a bit concerned when 1st heard American Board of Surgery was nixing ABSITE %ile reporting.
Have since learned its a solid evidence-based move w preserves %correct & standard score w are⬆️ accurate for formative & predictive uses while removing the most misused number: the dreaded percentile.

A study in the April issue of Journal of the American College of Surgeons (JACS) examines how home call results in sleep disruption and increased feelings of burnout—even on nights during which the surgeon was never called.

🎥 Lead author Jamie Coleman, MD FACS offers additional details about the study. brnw.ch/21wIpmg

Systematic sampling used by national surgical QI programs does not provide a robust evaluation compared to universal review and may not provide stakeholders with the data needed to inform local quality improvement efforts. ja.ma/3VmaGwo

A lot of discussions about difficult cases seem to assume the existence and availability of The Best Surgeon In The World For This, who is perhaps sitting there on the shelf in a lyophilized state, ready to be rehydrated and put into action when needed.

Many conditions treated by surgeons first require primary care screening and referral.

Dr. Sara Schaefer this week in Health Affairs connects primary care shortages to higher rates of emergency surgery for conditions that should be treated electively

❇️Best of Trauma❇️

This study of patients w/ blunt solid organ injuries demonstrated that VTE chemoprophylaxis initiated within 48hrs of arrival was associated w/ reduced rates of VTE without any observed increase in clinically relevant bleeding

Trauma patients often do NOT have satisfactory DVT prophylaxis with the commonly-used 30 mg BID dose of enoxaparin.

What if you adjusted the dose based on anti-Xa levels?
What doses do they end up on?

Check out the study by Grace Martin Niziolek et al:
