Some meds in hospital are ordered BID (9a, 5p) or TID (8a, 1p, 5p) when more appropriate may be q12 (9a, 9p) or q8 (e.g. 7a, 3p, 11p).

Certainly true for antibiotics, cardiac meds etc, but can be important for symptomatic meds too.

Example: still neuropathic pain at night:

Love this ECG from Willy Frick . RBBB in a patient with dextrocardia.

V1 looks about the same as it should in a levocardic RBBB.

But the deep, wide S, typically seen in lead I, is instead in aVR.

And lead I has another RSR', kind of how aVR normally would in RBBB.

Love this point: RV is anterior ventricle.

The reason we don't see STE in V1-V3 more often from RCA OMIs is that there's usually a simultaneous posterior-directed injury current from PDA territory of the LV, thus canceling or even 'winning,' and resulting in STD V1-V3 instead.
