We studied mortality in 450 members of cohort. All were homeless & 50 years old + when we enrolled them (most enrolled 2013-14)
26% died by 12/31/21

Thanks to a great team Zuckerberg SF General Hospital & Trauma Center! We designed and implemented a discharge checklist in patients hospitalized for HF. This simple, inexpensive intervention led to increased📈 GDMT rx and significantly reduced 📉readmissions in a safety net setting!
