Thanks to a great team Zuckerberg SF General Hospital & Trauma Center! We designed and implemented a discharge checklist in patients hospitalized for HF. This simple, inexpensive intervention led to increased📈 GDMT rx and significantly reduced 📉readmissions in a safety net setting!
We implemented a checklist Zuckerberg SF General Hospital & Trauma Center to promote evidence-based heart failure care. It reduced readmissions risk by almost 50% among diverse people.
